I am writing in regards to the employee disciplinary meeting you attended on [date]. I am informing you that the outcome of the meeting is that the company is issuing you with a warning/final written warning.

The warning has been placed on your file but will be disregarded for disciplinary purposes from [date] provided we are happy that your performance improves and the necessary action is taken by yourself.

To summarize;

The nature of the unsatisfactory contact/performance was: [nature of conduct]

We expect the following improvements on your behalf: [improvements] by [date]

The likely consequences for you if these improvements are not met will be termination/dismissal.

I want to inform you of your right to appeal this decision in writing to [person to appeal to name] by [date of last day of appeal]
